The Prophy/Recall Label is the basic prophy
label to document most prophy procedures. Some offices may
wish to use this simple label and others may choose to use the Periodontal Recall Label. Some offices will use both--the Periodontal Recall Label for
root planings, periodontal recall visits and more involved cases and the Prophy/Recall Label for routine prophy procedures. Size
4.5"x1".



The Medical History Update Label is reviewed and signed by the patient. This label is usually used for
a patient who has not had a prophy or been seen recently. Size 4.5"x1".



The Informed Refusal Label is for those patients who refuse treatment. They will
sign the label for medico-legal purposes. Size 4.5"x1"

The Observed Teeth Labels ensure that watched procedures are not forgotten. When a charting page is completed, the observed notes are copied to the label and the label is placed on the next page on the right hand side of our
forms. Size 2"x2".

The Simple Treatment Plan is used on the
right hand side of our charting pages for simple treatment plans. The procedures
and fees are recorded and discussed with the patient and the patient signs
the label. Size 2"x2".
